Most NYSHIP enrollees never find out because the benefit is real, but the process isn’t obvious, and most massage providers don’t know how to handle it.

Does NYSHIP Actually Cover Massage Therapy?
Yes. The Empire Plan, the primary health insurance option under NYSHIP, covers massage therapy when it is provided as a medical massage by a licensed massage therapist and meets certain conditions. Coverage falls under the plan’s Medical/Surgical Program, not the basic hospital benefit, and it is subject to your deductible and coinsurance, like other outpatient services.
The key word is medical. A relaxation massage at a day spa without documentation is not covered under the plan. A therapeutic massage performed by a licensed therapist for a diagnosed condition, chronic back pain, muscle tension from a documented injury, postural issues, or soft tissue problems is what the plan is designed to reimburse. The service has to be medically indicated, and the provider has to be licensed in New York State.

Do I Need a Referral or Prescription?
This is where a lot of people get stuck. Under the Empire Plan’s out-of-network provisions, you generally do not need a referral to see a licensed massage therapist for medical massage, but a physician recommendation or prescription documenting your condition strengthens your claim and is often required for reimbursement. If your doctor has told you to try massage therapy for a specific condition, get that in writing before your first session. Even a brief note in your patient portal visit summary works.
If your provider has told you to try physical therapy or massage for back pain, neck tension, a muscle injury, or a postural issue, ask them to note it in your chart and provide documentation. That paperwork is what turns a spa visit into a reimbursable medical expense under your plan.
Is Island Salt & Spa an In-Network NYSHIP Provider?
We are a licensed massage therapy provider in New York State. NYSHIP Empire Plan enrollees can submit claims for out-of-network reimbursement directly through their plan. We can provide a detailed receipt and session documentation that includes everything your plan typically requires: the therapist’s name and license number, the date of service, the type of treatment, and the amount paid.
To understand your specific in-network vs. out-of-network reimbursement rate, call the number on the back of your Empire Plan card or visit the NYSHIP portal through your employer’s HR system. Reimbursement percentages vary by plan tier and whether you’ve met your deductible for the year.

What Conditions Does NYSHIP Cover Massage For?
Coverage is tied to medical necessity, not a specific list of approved conditions. That said, the conditions most commonly documented for Empire Plan massage reimbursement include chronic lower back pain, neck and shoulder muscle tension from repetitive strain or postural issues, headaches with a musculoskeletal component, sports injuries, soft tissue injuries, and recovery from orthopedic procedures.
If you work a desk job, commute on the Sunrise Highway or 495 corridor, and have been carrying neck and shoulder tension for years, that’s a real clinical picture. A lot of our clients in the Bohemia and Oakdale corridor work in healthcare or school administration and have exactly this profile. The condition doesn’t need to be dramatic to be documentable.
What Should I Bring to My First Appointment?
Bring whatever documentation you have. If your doctor gave you a recommendation or a note, bring that. If you have an explanation of benefits from a prior claim, bring that too, so we can see what format your plan expects. Bring your insurance card so we have your member ID and group number for the receipt.

How Do I Submit the Claim?
Empire Plan out-of-network claims are submitted through the Empire Plan’s claims portal or by mailing a completed claim form with your itemized receipt. Log in to your NYSHIP member account at empireplan.com or contact Empire Plan Member Services at the number on your card for the current submission process. Most claims are processed within a few weeks of submission.
Keep a copy of your receipt and your claim submission confirmation. If your first claim is denied, you have appeal rights under the plan, and in many cases, denials are reversed when the medical necessity documentation is clear.
How Many Sessions Does NYSHIP Cover?
NYSHIP does not publish a fixed session cap for massage therapy, unlike some plans that cap physical therapy visits. Coverage is subject to medical necessity review, and your plan’s annual deductible and out-of-pocket maximum apply. Practically, clients who submit claims with consistent documentation, same condition, same licensed provider, and ongoing treatment tend to have fewer complications than those submitting one-off claims without a clear treatment picture.

Can I use NYSHIP for a couples massage or a relaxation massage?
No. NYSHIP covers massage when it is medically indicated and performed as a treatment for a specific condition. A couples massage or a general relaxation session doesn’t meet the plan’s medical necessity standard. If you’re coming in for a medical massage under the Empire Plan, it’s a separate booking from a spa relaxation experience, and the documentation reflects that.
Do I need to tell you I’m using NYSHIP before I book?
